Founded in 2016 through grant funding from The Nicholson Foundation, the Health Coalition of Passaic County has successfully built a thriving community coalition and is one of four New Jersey regional health hub organizations. Our mission is to significantly improve health equity, health and overall quality of life for residents of the greater Passaic County area by specifically addressing social determinants of health. Health starts in our homes, schools, workplaces, neighborhoods, and communities. HCPC is working hard to improve health outcomes for Passaic County residents.
