Founded in 2012, City Health Works demonstrated the power of health coaching in delivering exceptional results for patients with chronic conditions. In 2020, City Health Works achieved its mission of proving the business case for health coaching for individuals with poorly controlled chronic conditions. In response to the impact of the outbreak of Covid on the communities we served in Harlem, City Health Works wound down operations and its team launched a new entity, CHW Cares, to more robustly meet its mission.
