The CHI Health Partners network is a selective partnership of independent and employed health care providers, collaborating with hospitals and ancillary facilities to improve patient outcomes while lowering medical costs. CHI Health Partners has chapters in Grand Island, Kearney, Lincoln, and Omaha in order to give providers the opportunity to impact decisions that affect health care in their communities. We serve our more than 2,700 provider members with services including value-based care contracting with payors, provider credentialing and continuing education. Patient care coordination services help to improve patient outcomes and lower overall medical costs by reducing readmissions, ED visits, and duplicative tests.
